My guess would be that in a confined space, such as a locked car on a hot day, the amount of Oxygen would be rapidly depleted. It does say that it only takes an hour for the inside of a vehicle to reach 48C, when the temperature outside the vehicle is 20C, so my guess is the child may have had heat stroke.
