Somebody messed up but this stuff happens all the time, though usually on a smaller scale. Heck just a couple weeks ago we upgraded a major part of our plant with a new part from the oem manufacturer and even a consultant to help us. A major peice for whatever reason wasn't even close to fitting and required some major modifying to make it work. Haha still funny.
