I think it's a mistake to assume that every situation would be leaked on the media or internet. Every year players become free agents and many of them have clauses. We wouldn't know if it was a case of them refusing or not. Sundin was a high profile player, so that could be why it was so public.
The above is true for any player, free agent or not. I am sure we don't hear about the vast majority of trade proposals for trades that never happen, no matter what the reason.
Also, Kiprusoff apparently refused a trade as well.
