As with many rock stars, yes, but it he seemed to have put it behind him.
This interview published Oct 21, 2014 (likely his last) suggested he was clean currently.
This press release from his publicist says
"This is not a drug related incident or an O. D...".
In the immediate aftermath of the announcement one of the guys from Papa Roach posted on Instagram suggesting it was an OD, but he's since the deleted the post.
So who knows, maybe he relapsed, maybe he just did too much damage over the years and despite being clean it caught up with him. Or maybe he just had a bum ticker that gave out early, regardless of his past habits.
