The problem with 'bony girl arms' is the 'girl' part.
By saying bony girl arms, you are actually saying that girl arms are bad. There are also bony man arms. Or we could just go with 'bony arms'.
The answer to the question is no, it was not just a fat joke. One might assume it was a fat joke just like they would assume 'bony girl arms' was a skinny joke. However, bony girl arms is not just a skinny joke, it's an insinuation that it is lesser to be a girl and have girl's arms.
