it is perfectly OK to do so, but is it courteous?

Ummm, what? I'm supposed to magically infer that you have bad knees - bad enough that you need extra space to get into the car but NOT bad enough to prevent you from parking as far away from the doors as possible - and need extra room to get out of your car, and therefore it's only courteous for me to not park beside you?

The only reason I prefer more open parking spots is because my vehicle is large and I'm not that good at parking. It's not because I feel entitled to the extra space to open my door and get out. I've hit vehicles by accident in parking lots before and I'd prefer to not do that again. So... Yeah. I do actually think I'm being courteous by actively trying to mitigate hitting other vehicles.
