It looked like neither Brazil nor Chile ever practice spot kicks. Just some awful efforts there period. Soft shots down low either straight down the middle or close to it.
Don't watch club soccer only international so a bit naive about this but penalty kicks seem just like free throws in basketball - can't understand why these aren't almost automatic.
Why are players still kicking it low, soft and right at the keeper? Have your 5 designated guys who practice them religiously and pump them high and hard. Unstoppable shot like that one Chile player did.

Couple of reasons
Fatigue and pressure: those players were very tired and there was just a little pressure on them
and secondly, there are very little margins kicking a round ball. That unstoppable shot by the Chilean player could have easily gone over the net. The last Chile shot that hit the post wasn't dis-similar from it.
