I don't know what the statistics are on accidents vs. gross negligence. What they're saying though is that not one single death has resulted in anyone paying the price. I'm sure that at least some of the deaths are the result of gross negligence.

Part of it is probably a lack of workers informing an authority of gross negligence for fear of losing their job. In the Westray disaster, there was no where else for those people to be employed in town so they just dealt with things as best they could. That should never be a reason for keeping quiet.
