It generally refers to a premeditated crime, something done with some thought as to how to do it and some follow through.

It's opposite is a "crime of passion" in which someone acts because their emotions are so high they aren't in control of themselves.

Generally, if people are "hot headed" or "hot blooded" they do things without thinking, especially of the consequences. If you do something in cold blood, you are well aware of the consequences, but choose to do the action anyhow.
