I have found, and i think this was advice from Shawnski, that if you are exp grinding and can't one-hit-kill trash mobs, you are likely too high of difficulty for maximum efficiency.
I think the same holds true for farming, but I would give a bit more flexibility to that if I was trying to make the jump to T1.
My DH and Wizard can farm T1 really easily, but my DH is a bit more fragile. My wiz can do T2 without much risk of dying, but it is slow enough that I don't bother.
If I try with my WD or Barb (lvl 67) it is painfully slow, but both can survive.
Right now I am leveling my Barb and I seem to have found a sweet spot in Expert Adventure Mode. Nothing is a real challenge and even my damage impaired Barb can go through with relative ease. I suspect I will continue that with my monk and (eventually) my Crusader.
Kind of funny, I ended up killing Malthael with my Barb when he was only level 60, as a bounty. He was pretty tough, and I ended up killing him with a bleed after he finished me off. I tried again after I had leveled up to 66 and he wiped the floor with me. The difficulty scaling is very odd that way, I don't remember it being that bad pre-60.
