I am tending to think that your first scenario is the most likely, but coupled with a lack of oxygen.
Perhaps an uncontrolled electrical fire that disabled the backup oxygen system. Or a sudden decompression event. Either way, within a minute or so, everyone and everything is knocked out, but the plane keeps flying until it runs out of fuel or hits something.
