Well yeah they kind of are. They may not want to be, and may be willing to change and that's great. But if they're uncomfortable being around someone because they're gay, the person is homophobic. Just like I would be Acrophobic if I was scared of heights. If theyre uncomfortable being naked in general thats different.
Homophobia does not only apply to people who are outspoken about it.
