Check the Device Manager (under System Properties/Hardware tab) and see if it is reporting any issues. You may also want to check the Event viewer (I'm assuming XP or Win2K).
I'd be tempted to remove the device, then let windows re-add it. This sounds like a driver issue.

It is onboard sound in a Toshiba Sattelite A70 laptop.
I checked the event viewer and the only thing remotely suspicious around the time the sound went down was the computer shutting down while a profile was using the registry.
I removed the device from the profile and restarted letting windows find the driver. The device wouldn't start up so I ended up doing a system restore in hopes of fixing the problem. It didn't work.
Help me, Obi wan Kenobi. You're my only hope. lol
