Unfortunately, a lot of people who are grossly obese self medicate with food. i.e. Food is the only thing that makes them feel better. This often leads to a cyclical problem as depression can set in regarding the obesity and then they self medicate with more food.

The problem with a lot of psychological issues is that moments of clarity often do not occur until the person's life becomes unmanageable. With all of the psychological issues that could have resulted in morbid obesity (pathological compulsion, etc.) it is most likely that the gravity of the situation is not grasped until prevention isn't an option.

Unless the person subscribes to the Homer Simpson school of thought there is little chance that someone uses this kind of logic.
