The bottom line is that I would always tip the driver. Not only do they have a greater chance of making or breaking your meal (getting lost, driving slow, etc)- in addition to waitresses they also have to pay for gas and vehicle costs. Especially if it's somewhere you order from often. The experienced drivers know who tips and who doesn't. Once again, that can mean your order comes quick and hot, or slow and kinda warm.
I have had 2 bad delivery drivers (very late, cold pizza, pizza stuck to the top of the box, left pop in the car) have the audacity to complain about not getting a tip. They ####ed it for the rest of the delivery drivers that come to my house. I typically try and pick up my food now anyway since it is faster and a lot of places offer a discount for in store pick-ups.

I have a hard time justifying tipping a delivery driver when a lot of places charge a $5 delivery fee. I realize that the driver doesn't see much if any of this money but that really isn't my problem. I am already paying extra to have it delivered, now a tip in top of that? No thanks.
