I am in the group insurance industry (some days it is so much fun, I fail to understand why they pay me) and this type of stuff varies by the insurance company/plan administrator. Also, sometimes plan sponsors (your employer) will pay extra to have thier logo on the drug card, or for cards issued with the name of the spouse.
My recommendation is, go nuts!!!
