I stayed with a well off family in Johannesburg in 2010. The guy worked for Panasonic and they didn't have wifi. Most of the people we stayed with used 3g usb sticks, or ADSL, but they bought their data a few gb at a time as they needed it. They didn't leave their wifi on constantly. While it's probably improved since then, I doubt their networks are as good as we have it in North America.
I'm sure there are several countries we don't think of as being part of the third world where this is a potential issue.
