Honestly, no. And from past threads on here I don't think CPers really know anybody in the residential landscaping business.
You may just have to piece this together yourself. I'd call Brian's Bobcat and get them to rip out your old sod and 4" - 6" of dirt. Then I'd get Burnco to deliver loam. Then I'd rake it and roll it out myself (rent a drum roller and landscape rake). Call Blue Grass and have them deliver sod then lay it yourself.
