Some carriers will use a different APN for tethered mode, others will do deep packet inspection and look at the browser identifier header on HTTP requests, they can examine TTL on the packet (since it goes one more hop from the originating device, via the phone, to the network), etc.
There's a bunch of ways they can figure it out.
