Jefit. Its free and wicked. Has tonnes of exercises, tracks everything you want to track and like I say the office is right. You can go to their forums as well and find different routines which will then sync to your profile, and of course you can alter these routines to adjust to things you like/don't like about their workouts.
I'm pretty cheap and hate paying for apps (apologies to the developers here, and I do after I see the quality!). I used jefit for a couple weeks and paid the $4.91 for the pro version. I would tot rally recommend this app though, they've done a great job!
