There is only one area that a dog should be off-leash and that is a designated park (I've even admit in another dog walking thread I was a little misguided about dogs being off-leash) and never have I walked around where a dog was just running up and around communites, unless it escaped from the yard or something.
That is in 3 communities of Bridlewood, Varsity and Mission.

Depends on your definition of irresponsible. A dog barking more than once is irresponsible to me, but I'd guess you guys find barking perfectly acceptable in your neighbourhoods.
A dog coming up to me and my family is something I would consider rude on the part of the owner (leashed or not, I don't want a dog touching me), but I'd guess you wouldn't find that irresponsible, either.
An owner that lets their dog take a crunch on the ground and doesn't pick it up is irresponsible.
An owner that lets their dog urinate anywhere other than their own back yard while within the city is irresponsible.
Basically, I think it would be impossible for us to agree about what we find irresponsible. IMO, every dog owner is irresponsible as they would be guilty of probably all of the above (I think there would be some that would keep their dog away from my family, however, which is always appreciated).
