Or are the other cities in your region simulated and not real people? Or maybe you have the choice of either?

You have the choice it seems like.

You can set your region to private and never interact with other people, or you can play exclusively with friends or join a public region.

Who knows what the public region is, whether you connect to it every time or what. It sounds really cool, but I'm a bit skeptical on how well their algorithms work.
Another thing is this:

Players will be able to specialize cities on certain industries, such as manufacturing, tourism, education, or others. Each of which have distinct appearances, simulation behavior, and economic strategies.[24][35] Players have the option to heavily specialize on one or build multiple specializations in any given city for diversity.

It SOUNDS like a button you press, like a skill tree where you specialize in a specific industry. That would be stupid, because I'd rather have my specialization be decided based on my planning decisions and policy decisions rather than clicking a button. If I wanted education, maybe I build a bunch of universities, tourism I'd have casinos and landmarks.
I dunno. Maybe someone that's in the beta can say something about it.
