I wouldn't say that someone with dementia wandering away is unrelated to the reason that they were admitted - in fact it is likely closely associated with the reason that they were admitted. As for the specifics of the case no one at the current time can really go into any details about them, was the guy on a pass? Did he run away from the program? Did they look for the guy? Who knows but the hospital is literally steps away from the reservoir so it isn't inconceivable that someone could run away due to confusion, fall down the hill into the water and drown.

As for choking on food most meals in mental health units are done in community settings such as large dining areas as per Milieu Therapy so I am thinking that there may be more information to come out at least before we start calling some people to be fired.
