I guess that is where the whole "wearing a mask" thing comes into play.
He comes off as this spoiled, arrogant, drunk rich kid. Who would think that Bruce Wayne, who can't even sit on his own board of directors, would be able to pull it together well enough to be Batman?
Bruce Wayne becomes an act more than the Batman does as time goes on in his world. He begins to identify more with one than the other. Which is why when the Batman is no longer needed he becomes this recluse that seems to have lost his identity. It also explains that when a chance to take up the cowl shows itself again he is so eager.
