You have to understand the cultural side of it though. This is primarily a dish served in Hong Kong, and is usually with fish. It's all about retaining the freshest ingredients, and such a way to serve it is, while gross to most Westerners, a common and normal meal in China. Even in Japan I was served (and I ate) the sushi of a live fish. It was an odd experience but keep in mind that the fish is mostly dead, it's really just nerve endings twitching; the body and brain has been paralyzed for the most part.
