The only time it strays are if the pitcher came into a game with the tying run in the on deck circle and records the final out of the game. So if there are two outs and the bases are loaded in a 8-3 game the pitcher would get the save.
Also the old time save of relief pitcher comes into the game with the lead and throws the final 3 innings.

When Santos came in the game for the home opener it was a 3 run lead, but he didn't get the save because he only recorded the final 2 outs. If there was a runner on when he came in, then it would have been a save since the tying run would have been on deck.
Odd rules. I never actually knew them until today when I did a quick google search.
