For the times when you don't get the counter icons, try and just jump over the guys instead. It doesn't allow for the monster combos as easy imo, but in the game itself it won't matter. I've only run into one instance in the medals where you HAVE to fight without counter icons for points. It's obnoxious, but at least it's only one.

I can get huge combo's (like 60+ with a big group), but the one I had problems was the damn Assassin's one where all the chicks have swords. I hate countering knifed weapons.
You need a lot of patience for those challenges as you basically need to go Perfect Knight to get the 3 gold medals for most of them.
