so Mr. Freeze creates two cure vials. one he destroys before fighting Batman, the other is stolen by Harley Quinn (and then Talia al Guhl somehow). Talia gives the cure to Batman at the end, he drinks half then the other half is destroyed when Joker attacks him. so what about all the people in Gotham that Joker infected with his blood? they were apparently in the thousands, yet are never mentioned in the end. are we to assume they all just died?

I believe that Oracle tells Batman that they were able to use Freeze's information to start synthesizing enough of the cure that they'd be able to save all the infected people because their infections weren't as far along as Joker and Batman were at that point.

I don't remember all the details (and really who cares about all the other people?) but I have a vague recollection of some sort of "everyone is going to be okay" message in passing at the end.
