Are you sure it's not because of your lack of willpower?
A hundred different things going on around you in that restaurant and you choose to continue thinking about that injection.
Being discreet does not make it entirely invisible to everyone. There is a chance that someone will notice his insulin injection despite taking reasonable measures to not draw attention.
Unlucky for you, you noticed. You have to think about needles and how selfish this diabetic was for not injecting himself in the washroom or in the car. Afterall, all this diabetic have to worry about is living a shorter life than the average person, the possibility of loosing limbs and weakened organs, having a weaker eyesight, financially supporting a never ending cost for insulin and supplies, seeing medical specialists more frequently, worry about their diet and sweets intake.
