That's fair, which is why people need to use discretion. Can he take his dinner into the other room? Not really feasible. Can the injection be done in another room? Maybe, if that's feasible it seems like a pretty easy accommodation to make.
Part of asking people to show discretion is understanding that in some situations there aren't better options, and that's when you have to show discretion and make the best of it. It is definitely a 2 way street.
