He wouldn't be in danger of passing out. Insulin is used to lower an impending blood sugar spike(from the food). Naturally, insulin occurs in the pancrease and is released after we eat. Its job is to allow your liver, muscles and fat to absorb the sugar in your blood, turning it into glycogen and store it for later. A diabetic will inject insulin in anticipation of a blood-glucose spike. A bigger concern would be low blood sugar, and insulin would not be used in that instance.

As for this situation; it doesn't bother me but I can see how it would for others. I imagine most people would go to the bathroom to do it. Could be in this case the bathroom was occupied? A friend of mine is diabetic, and he'll inject at the table but not before asking if it's cool and doing it extremely discreetly. Most people wouldn't notice if not asked.
