Wow, you know you live a cushy, worry free lifestyle when one of you major beefs is having to smell smoke outside and coming off of peoples clothing. Unreal! Sorry folks, people outside in the world can smell however they like and smoke as many cigarettes as they want, your personal preferences do not supersede law.
Now, if someone is smoking inside of the 5 metre zone outside of a doorway I agree, that is not cool, it is against the law and that law is made very visible in most public doorways. But if you think you have the "right" to not smell smoke coming off someone else in an elevator, your not only dead wrong, you've also got some serious entitlement issues.
