I think Dexter is smart enough to know the difference between not feeling bad about, as opposed to actively enjoying, killing. Deb is fundamentally a moral actor who has very definite ideas about right and wrong - which is why it bothers her so much that she doesn't feel any remorse about killing someone, as she thinks that she should.

He won't ever (of his own free will) reveal himself to Deb because he knows that she would view him as irredeemably evil, in which she would be absolutely correct. That's why I find her the most admirable character in the show, as she is the one who always tries to do the right thing, and judges herself no less harshly than she judges anyone else. Dexter is her opposite - the man who enforces judgement on others despite being no better himself than those he judges.
