Bravo Two Zero.
They even have stoppages! Can you believe that? They are doing essentially a section attack on an Iraqi position and the LMG gunner yells out "stoppage" and clears his weapon and then continues on. I was amazed when I saw that. Simply fantastic!
Here, starts at around 40 sec.

The main one that bugs me is pulling the cocking handle back when a new magazine is put into M/16/M4/C7 rifles.
When the last round is fired the there is a bolt catch that holds the bolt open. When you place a new magazine in the rifle, you hit the bolt release which allows the bolt to push the top round into the bore to be fired. There is no need to pull the cocking handle again, but it happens a lot.
