Grade I is a sprain, with minimal disruption in the ligament that is disrupted in separated shoulders (making up the acromioclavicular joint). Grade II is a bigger, but small tear, and grade III is complete disruption of the ligaments and muscle attachments in that area. There are worse injuries (grades IV through VI), but those are rare.

Usually for Grade I and II injuries, the management is conservative (sling, cold/hot compresses, physiotherapy etc.)

Grade III usually need surgery but even that is controversial.
