I've always found M/M to have great customer service, so I would think this is an isolated incident (albeit inexcuseable... they should have called you back regardless).
It's important to note that Meloche isn't a public insurance company - you need to be a member of a group that has an agreement with them (university alumni associations, employers, etc.). The reason they have the lowest rates is that they are quite selective about whom they insure... (including those with those previous claims). This may explain why they didn't return your request, although I agree they should have informed you of this!
