Are you sure that most Cash fans wouldn't have known who Trent Reznor was after Hurt came out? I would assume that a high percentage of his fans during the American recordings were fans of "Grunge" or "Alternative" music. Most old school Cash fans had written him off in the late 70's, 80's. The reason for his huge success in the 90's wasn't because of his old fans coming back to listen, it was the younger generation realizing that he ruled and his new albums were amazing.

So most people that heard Hurt probably heard of or listened to NIN.
