Once you've had the surgery done, you've got to be very careful about what you eat as over doing it, or not getting enough water etc can cause some major problems.
It's certanly doesn't take as much work as running 10 miles a day and controlling what you eat on your own, but it certainly doesn't mean he didn't work very hard to lose all that weight.

Just because there are risks associated doesn't make it difficult.
As you can imagine, having a stomach something ridiculous like 1/10th the size of what it was really suppresses the appetite, since the filling of the stomach releases satiety hormones.
Not a fan.
