I used to climb trees for a living and work out of a fifty five foot bucket truck. You overcome fear by gaining confidence in your equipment and in your self.

I also took a hover-exit course to work with helicopters. One requirement was to climb out of the helicopter while it hovered at about fifty feet and hang by your hands off one of the runners while they lowered you to a couple feet from the ground where you would let go. The trick with Helicopters is to not shift your weight too dramatically because any shift you make the pilot has to counter to keep things stable. When the instructor was giving us the instructions he asked us if we would let go while hanging at fifty feet in the air. Of course the answer was "no". He then said "well if your not going to let go there really is no danger". He made a lot of sense. It was fun.
