Within the context of The Downward Spiral I think TR's version basically means exactly the same thing as Cash's version.

I think it's the age difference that I notice. I always saw Reznors as a downward spiral as you say but I see Cash's as sort of a sending off song.
I really like how Cash sang the song. Granted it wasn't really an artistic direction he chose to go in, but the fact that his health was failing and his voice was weak made for a very sincere sounding song imo. Not that TR didn't sound sincere, I just found I could relate to that version more than the NIN one.
