Default Need help making sticky labels

Other teams in our hockey league have little sticky labels with their rosters on it, so they can just stick the label on the scoresheet each game, instead of writing everyone's name out. I have an HP F4180 all in one printer. So is there a program I use to make these labels?...and I'm assuming I can get the sticker paper that has the appropriate sized stickers pre cut from wal-mart or something? Each one is about 2" x 4". Here's our scorsheet, the other team had one of these..

It looks simple enough to make, would it be done in Word, or is there some program you can download that has pre-made templates that fit certain sticker paper packages you can buy?

Thanks.
