I always calculate is as a minimum of 15% rounding the sum of the tip and bill up to the nearest dollar, and if it's good service, then obviously a higher percentage, rounded up. I do this regardless of whether I'm paying cash or debit. It makes no sense to me that a waitress wouldn't want me to round up, and would rather take less money ($.50 on average) for the tradeoff of not needing to remember (or write down) a couple extra digits.
