The Church is structured like the church was when Christ was alive. Christ is at the head of the Church and the Prophet is his representitive on Earth then the 12 Apostles, the Seventy etc etc. If something affects the whole Church, then the Prophet makes the call. If something only affects one Area, then the Seventy in charge of that Area is in charge of it. If something affects only 1 ward, then the Bishop of that ward is responsible. If it is something that affects only one family, then the father is responsible.
Everyone is responsible to all those above him, and as a result they are guided in their descions by the rules that have been set up and the spirit of inspiration that guides them to help those they are responsible for.
I hope this makes sense. In an effort to make this post as concise as possible, I am assuming alot of the terminiology does not need to be explained. If any of it is confusing, I would be happy to clarify.
