Last night's episode was pretty good, again I feel like it didn't have that "something" that earlier episodes this season have ("The Constant") ... but it was very interesting. Ben's sure not too happy right now and I have to wonder if Jack is capable of making an intelligent decision at this point - my assumption with Lapidus leaving the satellite phone was so that the 815ers would be able to track the freighter folk rather than because they want to be followed ... maybe I'm not understanding, though.

I think the surgery will be used as a plot device to force Jack off the island in later episodes. The scene with him risking tearing his stitches last night was IMO foreshadowing. Some kind of physical confrontation will erupt soon, Jack will try to help but his stitches will rip and he will be forced to leave the island.
I do agree the show is missing something the first few episodes of the season had. I think this largely has to do with pacing. The pace has clearly slowed down and things are being set up for some kind of large finale.
I don't think anybody really knows what the deal with the tracker dropped from the copter. If the pilot dropped it, it was probably meant to get the people onto the chopper and save them before the island is incinerated. If the mercenaries dropped it, it may be to lead them into a trap.
