This episode was a little less 'bang for the buck' than last weeks which I can't recall having a wasted moment. All the stuff with Jack's surgery was wasted because we knew he gets off the island, so there's no tension and no point showing it aside from highlighting that he still loves Kate.
The major noteworthy element from this episode is that at this point Jack obviously knows that he and Claire are related. I was thinking that he must be made aware of this before he leaves but with all the ghost visitors off the island there's no reason his "Dad" couldn't have told him later.

I dont know i think it did have some tension.. when Bernard and his Wife were talking before Jack's Surgery(when getting the Table ready) They were discussing how folks come to the island and get better... Bernards wife asked if everyone comes here gets healthy(Like she did) then Why did Jack get Sick?. it was sutle but there are some questions raised I think..
