I'm having pretty good luck with Dxtory, which is available guilt-free for $44, or in other means if you're into that sort of thing. I know Hanni had some luck with MSI Afterburner, which is truly free, but the vidcap stuff is kind of buried in settings. Its not only for MSI cards, but some video cards (mine) don't seem to work with it.
Using Dxtory, I'm able to capture my video and microphone feed, but not game sound directly. Some of it comes through the mic, so its not a big deal. The down side, is that it comes out to almost exactly 2 gigabytes a minute. I can understand how that's a problem for some people, and I'm not sure if it can be solved easily. One thing we could do is take a break every hour or 2 to allow people to compress their massive video files. I don't know much about that kind of thing, but I just use Windows Movie Maker and it takes about 50% of the play time to compress it by 8000%. So 80 gigs become 1 gig, which is still kind of a chore to upload to YouTube.
