1 dead, 7 injured at Ohio state fair from ride malfunction
When rides like these "malfunction" it always concerns me. Especially when I think similar rides are being ridden by friends and family at the Stampede.

Didn't a leg of the spider go flying off a few years ago at
Stampede. No one died but it could have been far worse

I always wonder how well these rides are maintained as they travel from place to place and how often they are fully inspected for safety. Not sure on this one if it was from poorly maintained equipment or just a freak thing but I'm surprised this doesn't happen more.

Alberta regulates rides under the safety codes act.
http://www.qp.alberta.ca/documents/Regs/2001_223.pdf
It references an ASTM standard for inspection F2783. Unforntuately you need a subcription to access that standar but in general ASTM standards are of high engineering quality and reviewed and edited regularly by industry and academic experts.
I looked for and found the stampede failure report. Cause was fatigue cracking which should have been caught by inspection.

Because it is still too early to figure out the exact cause. They are calling it a malfunction but it could just as easy be caused by negligence.
To me negligence is not the same as a malfunction when it comes to machines.

In situations like this I will assume it is a malfunction until an investigation turns up something else.

While all the rules and standards are in place, actually doing the inspection and the maintenance can be a problem. Things can get missed, or a guy just does a half assed job and you have a problem. A guy could take a glance at his machine and sign off that he did his weekly inspection or maintenance.
I highly doubt 100% of the rides are inspected by a third party every year. It's probably the owner of the machine who does it.

That's why I don't go on rides at the Stampede ever. I don't trust these things that can be slapped together in a matter of a couple of days and being run by 16-year old kids with no teeth who works like 18 hours a day for 10 days straight.....

Even though I generally feel safe on the rides, I've met a couple "carnies" over the years. The one thing they have in common is that they will not go on rides at fairs. I've heard this repeated unprompted multiples time.
Same with Totem employees not eating the free popcorn.

I think one of the Stampede rides had a significant malfunction this year that trapped the riders.
I've always had trouble with trusting rides because they're major pieces of engineering assembled and maintained by carnies.
But if you look at the actual safety records very few rides malfunction dramatically.
But that one looked like a assembly problem.
